Choosing the Right Incision for Your Breast Augmentation

One of the most common questions patients ask before breast augmentation is which incision option is best. Orange County-based, board-certified plastic surgeon Dr. Andres Taleisnik explains that while four popular incision options exist, only two typically provide the control and precision necessary to achieve the most natural and beautiful results.
Understanding Your Incision Options
The four possible incision locations for breast augmentation are:
- Inframammary: along the crease under the breast
- Periareolar: along the lower edge of the areola (the darker skin surrounding the nipple)
- Transaxillary: through the armpit
- Transumbilical (TUBA): through the belly button
Any of these four incisions may be used to insert breast implants. However, Dr. Taleisnik does not recommend the transaxillary or transumbilical approaches because these incisions are much farther from the desired location within the breast, making it more difficult for the surgeon to maintain precise control over implant placement. This also makes it more challenging to ensure optimal symmetry, positioning, and shaping of the implants.
Which Breast Augmentation Incision is Best?
Dr. Taleisnik favors the inframammary and periareolar incisions because they allow direct access to the breast pocket and provide excellent visibility and control for the surgeon. Both options make it easier to produce natural, well-balanced results with minimal visible scarring.
In choosing between these two, Dr. Taleisnik generally recommends the inframammary incision for most patients. One reason is that the inframammary scar typically hides beautifully within the natural crease beneath the breast. Another advantage is that this approach carries a lower risk of developing capsular contracture—a condition where the scar tissue around the implant tightens, causing firmness or distortion.
